Transcript Request Form for Graduated Students
This form is to be used by all former graduates/students of Windham High School for transcript requests.  You must answer the questions below and feel free, if you have more than one request, to use multiple documents.  All transcripts that are sent directly to the student must be marked unofficial.

Please note that if you need an official transcript, it must be sealed and mailed from the WHS Guidance Office to your school or business.
